The CYP Co-ordinator is a non-clinical role that supports children and young people aged 2–17 and their families to access the right help at the right time. Working with GP practices, schools, mental health services and community partners, the co-ordinator provides early advice, practical support and help with referrals to local services.

The role focuses on early intervention for emotional wellbeing, behavioural, neurodevelopmental, school-related, and family or social concerns. Acting as a single point of contact, the CYP Co-ordinator helps families navigate local services and ensures care is joined up across the system.


At present, this service is available to children and young people registered with the following GP practices in Uxbridge and West Drayton: Belmont Medical Centre, Brunel Medical Centre, Central Uxbridge Surgery, Hillingdon Health Centre, Oakland Medical Centre, Otterfield Medical Centre, The High Street Practice, The Green Medical Centre, Yiewsley Family Practice.
